Each member of an ensemble (duet/duo, trio, quartet) must submit a separate application form using the same unique ensemble name. For example, Mickey Mouse and Minnie Mouse are a piano duo. Mickey and Minnie would each individually submit an application form. Their unique ensemble name is Disney Duo. Mickey would submit an application form with the ensemble name: Disney Duo. Minnie would submit a separate application form, also with the ensemble name: Disney Duo.
